I, on 01/15/2026 accept that this Release of Liability (“Release”) is executed by the signed (“Participant”) in favor of Peak Fitness Pros, its officers, directors, employees, agents, and affiliates (collectively, “Peak Fitness Pros”). 1. Acknowledgement of Risks The Participant acknowledges that participating in fitness activities, including personal training, group classes, online coaching, and outdoor adventures (“Activities”), involves inherent risks of physical injury, mental stress, or other harm. 2. Release and Waiver The Participant releases, waives, and discharges Peak Fitness Pros from any liability, claims, demands, or actions arising out of or related to any injury, loss, or damage resulting from the Activities, including injuries, actions, or behaviors of coaches recommended by Peak Fitness Pros, both in-person and online. 3. Indemnification The Participant agrees to indemnify and hold harmless Peak Fitness Pros from any claims, liabilities, damages, costs, or expenses arising from their participation in the Activities or the actions of the coaches. 4. Assumption of Risk The Participant assumes full responsibility for any risks of injury, death, or property damage resulting from participation in the Activities. 5. Medical Consent and Insurance The Participant certifies they are physically fit, have prepared for the Activities, and agree to carry personal health insurance to cover any medical expenses incurred. 6. Governing Law This Release shall be governed by the laws of the state of Residents. 7. Severability If any provision is found invalid or unenforceable, the remaining provisions will continue to be valid. 8. Acknowledgment of Understanding The Participant has read and fully understands this Release, acknowledging they are signing it freely and voluntarily.
